Get Healthy Information and Coaching Service (New South Wales) The Get Healthy Service offers free telephone health coaching to all NSW residents 16 years and over. You receive up to 6 months of coaching calls to help you stay motivated and feel empowered to reach your health goals. University qualified health coaches including dietitians and exercise physiologists offer support relating to healthy eating, physical activity, reducing alcohol or achieving a healthy weight. My health for life (Queensland) Led by Diabetes Australia, My health for life is a free, lifestyle change initiative for Queenslanders who are at high risk of chronic disease – type 2 diabetes, heart disease and stroke. Delivered in six or more coaching sessions by a qualified Health Coach, the initiative is designed to help individuals discover ways of changing their day-to-day behaviour — that they can live with. It’s provided in a variety of ways, from groups to phone or online coaching, — locally, at home or at work — with culturally adapted versions. My health for life is funded by the Queensland Government through Health and Wellbeing Queensland. The COACH Program® (Tasmania) Delivered by Diabetes Australia in Tasmania, the COACH Program® is a free telephone coaching service for people at risk of, or diagnosed with, type 2 diabetes. Beat It Developed by Diabetes Australia, Beat It is an 8 week fitness program that involves moderate intensity aerobic, strength and balance based exercises as well as education sessions on healthy living. This life-changing exercise program can help you get back into a healthy routine to better manage your diabetes and other chronic lifestyle conditions. Beat It is for people living with, or at risk of, diabetes or other chronic lifestyle conditions who want to increase their physical activity levels. Find out more. Baby Steps Baby Steps supports women who have had gestational diabetes during pregnancy, to make healthy choices. It is designed to empower women to implement lifestyle changes and reduce the risk of type 2 diabetes. Other type 2 diabetes prevention programs delivered locally across Australia Life! (Victoria) The Life! program is a Victorian lifestyle modification program that helps you reduce your risk of type 2 diabetes and cardiovascular disease. Run by expert health professionals, the program is delivered as a Group Course or a Telephone Health Coaching service. AUS2PREVENT (South Australia) Diabetes SA, in collaboration with Flinders University, is testing the AUS2PREVENT lifestyle change program consisting of 14 face-to-face, structured group sessions and support over a 12-month period. The Trial aims to demonstrate that AUS2PREVENT is more effective at preventing or delaying type 2 diabetes and is also more cost-effective than current “Usual Care” (namely, brief education from the GP). Diabetes WA is adapting an online structured behaviour change program for Western Australian audiences at risk of diabetes and cardiovascular diseases. Let’s Prevent Digital is a free online program that will put you on the path to better health and wellbeing. It is a digital version of the face-to-face program. It’s not a fad diet or exercise regime. It’s a health program that provides you with practical information and skills that will enable you to make simple, sustainable lifestyle changes.
