We offer Medicare subsidised appointments, where you only pay a gap fee. Your GP must refer you to our clinic using an Allied Health Referral for you to access the Medicare subsidy. What do I need to do to prepare? To get the most out of your appointment, we recommend sending your diagnosis list, medication list and most recent diabetes pathology reports to our Patient Care team at [email protected]. This will ensure your health professional can provide detailed personal advice for your specific situation. Can I get a certificate of attendance for my appointment. Yes, our Patient Care Team is happy to email you a certificate of attendance. Please ask when settling your account. Why is the GP referred appointment cheaper? When a GP refers a patient to us, under a Care Plan using the Allied Health referral, the appointment is partially funded by Medicare. The standard fee is reduced by the Medicare payment and we only charge the patient the gap fee. What is an Allied health professional? The Australian Government defines allied health as “a broad range of health professionals who can treat conditions and illnesses. Allied health care is provided by trained professionals with university qualifications. They use practices with good evidence of effectiveness to prevent, diagnose and treat various conditions and illnesses. They often work in teams with various other healthcare workers to provide specialised support to patients.” When you are living with diabetes, relevant allied health services providers include health professions such as diabetes educator, dietitians, exercise physiologist, podiatrists and psychologists. People living with Diabetes, which qualifies as a chronic health condition, are entitled to a Medicare rebate on 5 allied health service appointments every calendar year. You access this via your GP using the Chronic disease allied health services referral form. Your GP will need to set up a GP Management Plan and a Team Care Arrangement.
