Diabetes Australia membership terms and conditions Diabetes Australia is an incorporated company and registered as a For Purpose public company limited by guarantee. Our ABN is 47 008 528 461. Diabetes Australia is owned by its members as defined in our constitution. Diabetes NSW & ACT, Diabetes Queensland and Diabetes Tasmania are wholly owned subsidiaries of Diabetes Australia. As a member of Diabetes Australia, you agree to be bound by the constitution and pay the relevant subscription/membership fee, and you consent to Diabetes Australia, Diabetes NSW & ACT, Diabetes Queensland and Diabetes Tasmania sharing your member information with each other. Membership Fee: Your membership fee covers annual membership of Diabetes Australia. Payment Methods.a. Payment online: there are several ways you can pay your Diabetes Australia membership fee. You can choose to pay an annual fee or pay by monthly instalments. To become a member online you must pay for your membership via a credit or debit card. You can use Visa, MasterCard or AMEX credit or debit card and you authorise us to charge the payment method associated with your account. If you are making a once off annual payment your payment will be processed on the day you join. If you choose to make an annual recurring payment the first payment will be made on the day you join and then on the 30th June every year. If you choose to make monthly payments, the first payment will be processed on the day you join and then monthly from the 30th July. If you renew your membership after the due date and choose to make an annual recurring payment the first payment will be made on the day you renew and then on the same date 12 months later. If you choose to make monthly payments, the first payment will be processed on the day you join and then on the same date every month.If using a Debit Card, it is your responsibility to have sufficient funds available in your nominated account by the payment date. You remain responsible for any uncollected amounts. If a payment is not successfully settled, due to expiration, insufficient funds, or otherwise, and you do not cancel your account, we may suspend your membership until we have successfully charged your debit card. We may re-attempt unsuccessful direct debits or contact you seeking instructions. Any dishonour fees incurred against your account by your financial institution are your responsibility.b. Call us to renew your membership. You can use Visa, MasterCard or AMEX credit or debit card and you authorise us to charge the payment method associated with your account. You can pay your membership fee via BPAY through your online banking account. You will need to use the Biller code 7674 if you were previously a member of Diabetes Queensland and the Biller code 127449 if you were previously a member of Diabetes NSW & ACT. The reference code is unique to yourself and you can find this on the printed membership form mailed to you or on your renewal email.e. Australia Post Billpay. You can pay your membership fee by taking your printed membership form, mailed to you to any Australia Post office. They will scan the barcode on your form.f. Updating your Payment Methods: We will use the latest credit card details you have supplied to us for all recurring payments with Diabetes Australia, Diabetes Queensland or Diabetes NSW & ACT. If you have previously provided a different credit card to us for recurring payments, please note that this will now be replaced with the new credit card details. To make changes to your payment methods, you can contact Diabetes Australia. Contact details appear at the end of this notice. Cancellation: You can cancel your membership at any time. To the extent permitted by the applicable law, payments are non-refundable and we do not provide refunds or credits for any partial membership periods. To cancel, defer or make alterations to your membership, please contact Diabetes Australia. Contact details appear at the end of this notice. Please note we require five days’ notice to process any adjustments. Changes to membership services and fees: We may change our membership services and pricing from time to time. You will be given 30 days notice of any change. Changing membership details: Only the named member, parent or guardian of a member under 18 years of age or person authorised in writing by the named member may request a change in membership details. Membership verification: All members are issued with a unique membership number. In circumstances where you are unable to remember your membership number, please contact Diabetes Australia. Contact details appear at the end of this notice. Voting rights: Members are entitled to attend and vote at Diabetes Australia General Meetings. For further information about your voting rights as a Diabetes Australia member please refer to the constitution. General Meetings: General Meetings are held annually, with Extraordinary General Meetings called as required under the constitution. Notices to members of these meetings will be provided in writing no less than 21 days before the meeting. Please refer to the constitution for further details. Promotional offer – 3-month trial membership: To be eligible for a trial membership with Diabetes Australia you need to provide your email address upon sign up for communications. During your trial period you will be emailed a digital Circle magazine, you can upgrade to a paid member at any time to be eligible for full member benefits including a paper copy of Circle magazine. If you do not choose to join as a paid member at the end of the trial period your membership benefits will cease. During the period of a trial membership of Diabetes Australia, you do not have voting rights.
