Get Healthy Service – New South Wales Want your own health coach to help you reach your goals? The Get Healthy Service is free evidence-based phone and online health coaching for people over the age of 16 living in NSW. It is delivered by Diabetes Australia on behalf of NSW Health. The program is designed to fit in with your life, with health advice tailored to your needs and goals. When you join Get Healthy, you’ll get six or more coaching calls on a range of health topics including eating healthy, getting active, reducing alcohol, reaching, and staying at a healthy weight. It can also be personalised to help manage your health while living with diabetes or cancer. There is also a program to support women during pregnancy and beyond. Get Healthy is available in languages other than English. Free interpreter services and the National Relay Service are available. If you identify as Aboriginal and/or Torres Strait Islander, you can yarn with an Aboriginal Health Coach and get information specific to Aboriginal communities. My health for life – Queensland Led by Diabetes Australia, My health for life is a free, lifestyle change initiative for Queenslanders who are at high risk of chronic disease – type 2 diabetes, heart disease and stroke. Delivered in six or more coaching sessions by a qualified Health Coach, the initiative is designed to help individuals discover ways of changing their day-to-day behaviour – that they can live with. It’s provided in a variety of ways, from groups to phone or online coaching – locally, at home, or at work – with culturally adapted versions. My health for life is funded by the Queensland Government through Health and Wellbeing Queensland. Diabetes Australia Health Coaching Service – ACT Residents of ACT can access Diabetes Australia’s telehealth coaching service, a free pilot program offering ongoing support from qualified health professionals. Helping people to manage their prediabetes and learn practical ways to reduce their risk of developing type 2 diabetes, through group education sessions and personalised follow-up support. Sessions are held regularly in Darwin, Palmerston, and Alice Springs, with individual consultations available for people in remote communities or those unable to attend a group.
